How to Join LEAH


  • Download Membership documents and all applicable forms:
    • Policy Statement
    • Application
    • Attendance Record
    • K-5 Waiver (Election Form for Non-Attendance in Complusory Education)
    • Request for Student Records (from public, private, or home school assn.; if coming from out-of-state, send Academic Record from home)
    • Foster Parent Custody Decree
    • Notice of Non-Custodial Parental Consent
    • Pending/Previous Government Case
    • List Additional Students
    • List SCVSP classes or Dual Credit Classes
  • Complete Application and documents, mail with payment to LEAH
  • Receive Confirmation of Approval for Membership and Member ID cards
  • Upon receipt of approval, download applicable forms:
    • Elementary or High School Proposed Course of Study (Grades 7-12)
    • Course Descriptions – 1 for each class to be attempted for high school credit
    • Instructions for Portfolio Checklist, Elementary or High School
  • Complete and Return Applicable Forms (see 4 above) within 30 days of approval of membership together with Tier Fee, Discount Membership Dues for SCHEA, or (HSLDA with completed HSLDA application)
  • Request for Confirmation of Membership in Support Group be sent from membership secretary to LEAH, LLC.
