How to Join LEAH


  • Download Membership documents:
    • Policy Statement
    • Application
    • Attendance Record
  • Complete Application and mail with payment to LEAH
  • Receive Confirmation of Approval for Membership
  • Upon receipt of approval, download applicable forms:
    • Kindergarten Waiver
    • Elementary Proposed Course of Study
    • High School Proposed Course of Study
    • Course Descriptions – 1 for each class to be attempted for high school credit
    • Release of Documents (for transferees from public or private school)
    • Academic Record Form
    • Instructions for Portfolio Maintenance
  • Complete and Return Applicable Forms (see 4 above) within 30 days of approval of membership together with Tier Fee.
